Re-issue of Passports [Normal: 36 pages & Jumbo: 60 pages]

• As per instructions in general guidelines, printout of duly filled online application form available at https://mportal.passportindia.gov.in/mission/, should be submitted at VFS centre. All particulars in the application should tally with those in the current passport. If there is a change in personal particulars, the requisite supporting document must accompany the application.

• Passport will be retained during the period of the service.

Documents required:

1. Duly completed and printed application form (https://mportal.passportindia.gov.in/mission/).

2. For ICAO compliant photographs please click here.

3. Current Passport in original along with self-attested copies of first (personal particulars), last (address) pages and any other endorsement pages and visa page of the passport and additional booklets, if any.

4. Self-attested copy of UK Entry visa.

5. Self attested copy of UK eVisa (online immigration status) along with share code or Indefinite Leave to Remain (ILR).

6. Annexure E [Download].

7. Undertaking where Passport is withheld/ kept with UK Home Office [Download]. 

8. In case of change in address, self-attested copy of address-proof like Election Commission ID card, water, electricity or telephone bill, nationalized bank passbook with 6 months bank statement (with permanent address mentioned in the passbook attested and signed by bank official along with applicant photograph) and Aadhaar card issued by UIDAI, Government of India will be accepted. These documents must be in applicant’s name. For minors, parent's passport copy will suffice to change address.

Documents required in Marriage, Divorce and Separated Case:

• For addition of spouse name in passport:

1.  MARRIAGE CERTIFICATE or

2.  Joint Photo Declaration signed by both husband and wife (Annexure J) [Download]. 

• For deletion of Spouse Name:

1. Divorce Order/Decree.

• For Change of Spouse Name:

1. Divorce Order/Decree or Death Certificate of first spouse and

2. Re-marriage certificate or Joint Photo Declaration signed by both husband and wife (Annexure J) [Download]. 

• For a change of surname by women applicant following marriage/divorce:

1. Marriage Certificate or Joint Photo Declaration signed by both husband and wife (Annexure J). [Download] 

2. Divorce Order/Decree (if name/surname change is based on divorce).

• In case of a full change of name after marriage, the standard name change procedure should be followed.

P.S: Applicants are required to furnish original documents along with one set of self- attested photocopies of the same at the time of submission of application.
